How do I get a copy of my DD Form 214 (Certificate of Release or Discharge From Active Duty)? This document was given to you upon discharge and hopefully it is among your personal papers. We suggest that if you do not have this form that you submit a completed copy of Special Form 180, Request Pertaining to Military Records, to the National Personnel Records Center (address below) to obtain one. Having your DD Form 214 will also be beneficial in the future when you apply for veteran’s benefits, Medicare, or for Social Security. If you do not have your DD Form 214, then you may download a copy of the SF 180 at http://www.archives.gov/veterans/military-service-records. Also, for your convenience we have archived the SF 180 on our site so you can download the form from this web site.
